An employee's Personal Information can be specified when creating a new employee profile, or editing an existing one. This article describes the fields that you can specify when updating that section.
- First, Middle, and Last Name
Specify the full name of the employee. This field is required.
Specify the Social Security Number for the employee. This field only is displayed when creating an employee profile.
Note: When creating an employee profile, you must provide a valid number in the SSN field to submit the profile. The CertiPay Online application does not allow you to edit an employee's social security number once the profile has been created.
- Date of Birth
Specify the date of birth for the employee in MM/DD/YYYY format. This field is required.
Select Not Specified, Male, or Female from the drop-down list. Not Specified is selected by default.
- Marital Status
Select Not Specified, Single, or Married from the drop-down list. Not Specified is selected by default.
